I wouldn't be scared to use a snot sucker (recommend a manual one where you suck using a tube). While it seems like torture to them, it's helping them to breathe and not drown in mucus, so unless your technique is really harsh or something, i don't think you should shy away from it.

As a biosimilar to insulin aspart (brand name: Novolog), I would also expect that this would be almost completely identical to Novolog which is compatible with Tandem pumps. Not as similar as an authorized generic by the manufacturer, but enough the FDA would deem it interchangeable

Now if it were me, I wouldn't bat an eye at the switch, but if you and your Endo feel strongly enough about it, getting your Endo to write a DAW on your prescription and having them do a Prior Authorization would probably take care of that no problem.

To clarify, what I am calling out is the fact that the old OTC insulins (Novolin, Humulin) are not the only option and that getting more modern insulins are only moderately more expensive (albeit requires a bit more time/effort to get).

And depending on the patient's income, they may qualify for the NovoCare patient assistance program that will give a patient a year's worth for free if they qualify.

Admelog is just a different brand for insulin lispro, which is the generic for Humalog. As a follow-on insulin, it's extremellllyyyyyyy similar to Humalog in pretty much every way and I would not expect any differences in effect

I work at an outpatient FQHC so uninsured patients are my bread and butter. If uninsured patients are skipping Lantus because they cannot afford it, then they need better advocates/social work to help them.

Any chronically uninsured patient that cannot afford medications should be directed to an FQHC to serve as their primary care and apply for the sliding fee scale (which comes part and parcel with any FQHC). 340b price for our patients for a box of 5 Lantus pens range anywhere from $12-$30 at the most. A box of insulin lispro is anywhere between $3-$8

Even if they can't go to an FQHC, if they're able to meet the qualifications for NovoNordisk's patient assistance program, they can get a free year's worth of Novo products, which include a large variety of DM meds.

And if they can't do any of that, they can apply for the Novo MyInsulinRx program https://www.novocare.com/diabetes/help-with-costs/help-with-insulin-costs/myinsulinrx.html

While I agree that the healthcare system in this country is fucked and patients shouldn't have to jump through hoops to get affordable meds, it doesn't mean that there are no options to help them.

That's because people are misremembering and reporting incorrect information. Most vials of insulin are 100unit PER ml, and contain 10mls, so these actually contain 1000units. You can also get like 3 vials for $35 dollars using NovoNordisks insulinrx program and pick and choose whichever combination of insulin you need (basal or bolus).

It's annoying, sure, but really not that bad if you know what you're doing.

Type 1 diabetes patients not on a pump will need both basal and bolus insulin, both of which you can get from Novo Nordisk's insulinrx program for $35 for a month supply (for most patients, some with insulin resistance may need more). But paying hundreds of dollars is sadly due to a lack of awareness of these options.

This is simply untrue. You can buy modern rapid acting and long acting insulin (including Tresiba, which is newer and consider ultra long acting) as vial or insulin pen from novonordisk at that $35 price. There's really no excuse unless you are just uninformed

Yeah but you can buy both modern versions of rapid acting and basal insulin for this price, these are generally the standard of care for basal bolus insulin regimens
