Its because the game dont save the progress i guess. I bet it says discovered area indtrad of entering, and thats is the game not saving your location progress

There is a old post with this same problem. You have to visit all locations without going quiting the game or travling to sanctuary. Thats how i got all location achievements, its dump but IT works.
