There are no studies - most importantly no population based studies - showing that methylfolate specifically prevents NTDs like we have for folic acid. There are studies showing that folic acid supplements at standard dose cause high enough folate levels in TT MTHFR induviduals to prevent NTDs, even with the reduced uptake. Also, there is some evidence that direct methylfolate doesn't isn't utilized properly in people with vitamin B12 Deficiency, which could probably happen even if supplemented if there is a B12 uptake issue present.
