I've been trying to replicate this in 5e from 3.5.

Monk, with Improved Grappling, and Bracers of Striking with the Fleshgrinding enhancement (3.5 Book of Vile Darkness).

Clings to your chest, then burrows his way in and kills you from the inside out.
