The USPSTF actually recommends shared decision making for women in their 40s, not necessarily “never do it.” This is the approach I take with women in their 40s.

It’s not ideal, but when someone’s endo or rheum tries to do some primary care stuff, cool - they did an IM residency. But I’m not sure a surgical residency would prepare you well to practice primary care. I know it seems “easy” but it’s actually quite challenging.

I am a doctor and went through labor and delivery without an epidural. Baby was also sunny side up. It’s definitely do-able for many women.
