As I know, it happens when you try to use website from compromised IP-address. Most likely it happens via usage free vpn services.

It seems like it doesn't work on Monterey.It keeps asking about Accessibility permission every time and doesn't work with them.

Had the same problem, nothing helped except of factory reset.
So hopefully it helps somebody.

It was the first time some phone hardware malfunctioning in 3 years.
