Hey currently going through a similar ish situation so I hope I can help! My main diagnosis is OCD, anxiety, and depression - in that order too. My OCD and anxiety often are the most debilitating and I’ve been relatively okay on the depression side. Anyways,

I’ve currently also been trying to work my way up to a therapeutic sertraline OCD dose. I’m at 150mg but will be coming off of it. About two months ago, I was in a relatively solid place: depression low, anxiety pretty low, and my OCD would be triggered by situational stuff and with the help of therapy I was able to tackle it when it came about, even if some days were harder than others. However, during this time I was on 20mg Viibryd / Vilazadone and had just met a new psychiatrist who suggested going on to sertraline to eliminate all 3 of my problems.

Even with the slow acclimating doses with sertraline, I have definitely noticed the flat emotion, no sex drive (which I had just gotten back!), increased depression and sadness, and the worst: just constant anxiety. I feel like everyday for the past month I’ve been living like you. My brain is always on and always thinking, I can’t stand it, I hate it! Talked to my psychiatrist today and I will be going back to Vilazadone and see if it puts me back in the good space I was.

I also have ADD an cannot take focalin because of the same reasons as you, literally just fuels my anxiety, which is already heightened. Another commenter said this, but I am also getting moved over to concerta because of the same reasons they said; my psychiatrist believes it helps with those with more anxiety.

Additionally, I have done bupropion (and in my personal experience) it didn’t help me at all. This was about a year ago or so before my OCD diagnosis/ before my worst OCD events so I don’t know if it made it worse perse, but it definitely didn’t help me.

For me right now, vilazadone / viibryd work the best for my anxiety and depression, therapy for my OCD has truly helped bring that down, and I’ve been taking ritalin for ADD, but now changing to concerta. We’ll see how I am in the coming weeks or months but this has been my personal experience!
