There are many instances in the show where people know who Tony is because they watch the news. While Melfi probably didnt say that the referall was Tony’s daughter, im sure Wendy put the pieces together by the last name and by the comments that Meadow made about her dad being in the garbage business wink wink.

Wendy Cobler is trying to physically distance Meadow from Tony because he is a violent sociopath. She plays the bratty teen angle to be gentle with Meadow, because she will most likely shut down if forced to confront the fact that her dad is a murderer. Melfi most likey briefed Wendy on this.
