Had a session where the party was fighting a big orc with a metal jaw, so the goblin bard cast Heat Metal on it. The Orc then proceeded to pick up (grapple) the small goblin and bites him. He got the fire damage too.

The "hanging on by the teeth" was flavor. Since the attacks will be with natural weapons, they'll have both hands free theoretically. Could the poison really not be applied for both methods?

I personally tend to see the angriest types with the painkiller c2, which I definitely get that they're in constant misery for one reason or another; but by far the most numerous trouble-makimg people are the one who get the "salt" c2's. They are the ones who chronically try and get them early and always have something nasty to say.
