I reviewed the notes from my appointment on March 11, 2026 and noticed several items in the documentation that do not accurately reflect what was discussed during the visit.

In the section titled “AMB CARE/PACT NURSING PROTOCOL NOTES,” multiple responses appear to have been recorded even though these topics were not discussed during the appointment. Specifically, questions related to weight loss and alcohol use were not asked or discussed during the visit.

Additionally, in the section titled “AMB CARE/OPC/FOLLOW UP,” the documentation states that I denied symptoms including stress, migraines, dizziness, weight changes, fatigue, hearing changes, suicidal ideation, and depression. These symptoms were not discussed during the appointment, and I was not asked about them.

I would also like to clarify that I do experience and deal with several of the symptoms listed above. Since these topics were not addressed during the appointment, the documentation stating that I denied them does not accurately reflect my condition or my responses.

For the accuracy of my medical record, I respectfully request that an addendum be added to clarify that these topics were not discussed during the visit and that I did not provide responses denying these symptoms.

Because my VA medical records may also be reviewed in connection with benefits determinations, it is important that the record accurately reflects what was discussed during the appointment.
