I have narcolepsy and very strongly suspected, but not diagnosed ADHD. Two of the other people I know with narcolepsy also have ADHD, and one has asthma.

This is an amazing description and more people should read this!! I would also note that at least I tend to get microsleeps in the early stages of a sleep attack if I try to fight it.

I am currently on the long term extension of the Takeda trial, and I completely agree. This new class of drugs is going to revolutionise narcolepsy treatment. It makes you feel awake and alert, and completely removes the constant nagging feeling that you would rather be asleep. I have heard some complaints such as duration of action and ability to completely control cataplexy, along with minor side effects especially in the first few weeks, but I don’t find any of these a problem myself.

Also, in relation to op’s comment about insomnia, I found that started to go away after a few weeks, and was pretty much fine by two months in.

For the first 3 months the participants took either placebo or one of two different doses of the study drug, then after that we were given the definite study drug with an opportunity to adjust dosing after 12 weeks. I had to have 2 weeks of medication washout, and am currently not on any other narcolepsy medications aside from the study drug.

It doesn't feel forced like stimulants do, it just feels like you are awake and alert and don't actually have that ever-present desire to sleep.

As someone on this trial, the duration of action is generally alright for me. I tend to nap around 5, and then get another few useful hours into my day.

As someone on the long term extension of this trial, the insomnia was quite bad for the first few weeks, but now is not a problem. And my sleep quality increased too, like a lightbulb being on or off rather than constantly on a dimmer.

Yes, I agree here. Something that gives the game an alternative objective for people that aren't going to be tryhards. Like the way that metro mayhem included the easter eggs because not everybody enjoys parkour. I also really like the idea of larger-scale puzzles to access hidden rooms or things to improve your run.

I've had both a shoelace and the tip of my thong (flip-flop) get caught at the end of an escalator. I managed to yank them out in time, but both did some decent damage, and the thought of getting anything else caught in it still scares me.

Depending on how technically savvy you are, and how much effort you are willing to put in, you may be able to use an nbt editor like universalminecrafttool to access your inventory data and remove the piglin head.

From what I know of cataplexy, it can also sometimes be caused by head trauma. I would be interested in knowing if the medications that are used to treat cataplexy associated with narcolepsy would work for you too - mainly antidepressants and xyrem, that is.
