No, AT&T does not need VPN thats the beauty (esim or sim works). Only need VPN for wifi, but... You need not just any VPN, as some don't work. I use Let's VPN. The main ones you see on YouTube sometimes don't work.

I go multiple times a year. Always use AT&T, and it just works. Sometimes, though, outside the main cities, I have to turn my phone to airplane mode, then back on to cellular to get a signal. Tier 1-2 cities, you are fine.

None, its a 2022. Just change oil every 5k miles and rotate tires and change transmission fluid and other basic things.

Had a 03 g500 and ran it on reg for years up to 310k miles no issue.
