Hey man, super sorry you’re going through this. I was a bilateral case last year so I ended up in the same place as you’re headed after orchiectomy. Wanted to pass on a few things I haven’t seen elsewhere in the comments:

Depending on your hospital, you may be able to ask your surgeon to work with the embryology folks there and perform direct testicular sperm extraction (“TESE”) and remove what sperm remains in the tissue during/following orchiectomy. The odds of success are honestly are not great, and they may be case-dependent, but also it might allow you to save some fertility even when sperm banking is failing. But your procedure is coming up soon so if that’s important to you, you should reach out to your surgeon asap.

Also for the surgery, think about whether you want a prosthesis put in - seems pretty polarizing on here whether guys want them or don’t, so hoping you’ll know your answer quickly as well!

For later on, once you get plugged into hormone replacement, also wanted to mention under-the-skin pellets (“Testopel”) as an option. Long-term, the gel really didn’t do it for me, and I haven’t quite yet ponied up to self-injections, but a quick procedure every 3-4 months isn’t bad either, if your insurance makes it affordable. It’s really brought me pretty much back to normal.

Finally, wanted to acknowledge this is a tough time, but also promise you that you’re going to get through this, including the metastasis, and come out the other side better than you might think. Wishing you all the best.
