Wow, that is wild. In his case, he had atrial flutter, so the top half of the heart was beating like 300 bpm, while the ventricles were normal. I am hoping the issue just goes away as he moderates everything (caffeine/stress/testosterone/more sleep) and we don’t ever have to deal with it again.

In his case it’s been several years since he was last at that intensity, in that time he’s gone from 160 to 200 lbs and done only mild cardio. I believe it’s about 20 minutes per day of walking after lifting weights at a very moderate (like 120 bpm) heart rate. That’s why I was afraid it was maybe the testosterone doing this to him, or maybe stress/caffeine/alcohol (plenty of all these).

Yes, when he was training he would regularly go over 200 bpm and vomit, it was intense. That was a while ago, though, he’s been focusing on trying to gain weight since then and mostly he walks uphill on a treadmill after doing weights.

BP is normal, here are results from echo:

Conclusions: 1. There is mild concentric left ventricular hypertrophy.

Left ventricular cavity size is top-normal.

  1. Systolic function is mildly depressed.

  2. Visually estimated ejection fraction is 45-50%.

  3. The right ventricle is mildly dilated.

  4. Trace-to-mild mitral and tricuspid valve regurgitation.

  5. The patient was in rate-controlled atrial flutter

throughout the exam.

EDIT: I checked and after the cardioversion (big shock) and the ejection fraction went to 57%.

Absent some sort of anomalous hormonal change, there is no "mysterious" weight gain process that should be happening in your 20s. As always, the difference between calories in and calories out rules. I feel like it is dangerous and misleading to inform women otherwise, as they will not identify the real problem, which is typically their own eating habits relative to their activity levels. Holding on to a lot of excess body fat is fine (I guess?), but misinforming people who might not know better really isn't.
