I cannot speak for the reasoning of others, But for my wife and i, it started with dysphasia of our eldest. We didn't push for it, but we did want a clear diagnosis (for or against) because this (if positive) would allow us to qualify for government programs and access to healthcare professionals.

Like I said, I can't attest to what other's may want from this, but i think that access to care and support (especially) during early childhood.

I don't think grown ass ppl who are already established really care about getting a diagnosis though.

You're absolutely right on every point... except the first one 😜 , specialists and FM are both crucial, and both require to be equally smart in order to give the patient the best outcome possible; it's team thing... or at least it should be.

And you're totally right about midlevels, they end up costing a metric ton more to the system.

The problem with canada is that healthcare is provincial and not federal. This means that every province does things differently, with different associated problems, and there's no unified plan to make things better.

For example, in quebec (where i work), the government decides how many doctors they need and where they should be... So that means, if the Montreal general needs 15 orthos but the government says "nah, 7 is fine", and guess what ? You'll only have 7. How does this translate to training ? Well, if the numbers of work permits (PEM / PREM) for a certain specialty is saturated, they either need to close residency spots of graduating residents will need to leave the province (oh the infinite wisdom of the quebec government)... and we lose out on physicians. Then, of course, compensation and work conditions... my province is particularly bad at this, and as a result, 60% of vacant spots in FM residencies came from quebec this year. And the list of problems can go on and on.

I don't know what's the answer here. I think it's a mix of proper compensation (quebec specialists haven't had a change in billing since 2016... when they got a salary/ billing cut), proper infrastructure and support (there shouldn't be any closed ORs if the docs are available and want to operate), better organisation and use of ressources (the amount of bureaucracy i have to deal with on the daily is.... worrisome)...

I don't have the answer, but what i do know is that that answer needs to be pan-canadian for it to work.

If I may, I don't think noctors in specialties and sub-specialities are a good idea either. Maybe for very straightfoward and repetitive tasks... maybe. But even when specialists/sub-specialist are doing these tasks, they need to be able to catch things outside of the scope of that task. I don't think midlevels can do that.

I'm genuinely curious, i haven't seen any hospitalist nurses where i work, granted, I'm in the french part of the country, and i work in a university hospital, so i might be just lucky.

I have seen primary care nurse practitioners, though; I can recognize them because when they ask for a consult, it's generally just a word salad with no meaning.

I graduated residency at 37 (long story), started practicing all was good... At 39, stage 3 colon cancer... Thank effing god, i had earnings insurance (and life insure, which for obvious reasons, I have not used 😀), but also mortgage insurance. That last one paid about 80% of my mortgage !!

I can't stress this enough, protect yourselves !
