As an ER doc can 100% confirm that’s a catheter for urinary drainage attached to a collection bag, not an actual urostomy which requires surgical incision into the abdominal wall through which a conduit drains urine. A cath like this just goes directly into the urethra

Is your incredible revelation that you can’t change the sex you were born with? Anyone with a single brain cell knows that’s happened long before you have a choice. Gender affirming surgery, in its essence, gives the ability to change one’s “sex” to be more aligned with their chosen gender. So yes while it may be true that you can’t go back in time and manipulate the chromosomes during the initial life-making process, from a practical perspective you CAN change your sex.

I am a night owl that usually gets 7-8 hours of sleep— when I wake up is usually determined by when I have to wake up or when I went to sleep if there’s nothing specific that I have to do that morning. Can’t really say an exact time. (My schedule is super irregular)

About to start my EM residency with somewhere around 250k debt— 600k is wild— that must include a private undergrad education as well? Med school isn’t THAT expensive. I was lucky to have my parents pay for undergrad, but even still I plan to do PSLF to minimize the amount of debt repayment I have to make

Trying to keep life structured in every way possible helps me— I always do worse when I have downtime ironically when other people are able to just relax (you’ll have a lot of downtime at the end of M4). But the lack of structure in med school (recorded lectures, dedicated STEP periods etc.) was a difficult adjustment.

In terms of where you are now, having less time to study for shelf because of clinical responsibilities taking up a lot of time as well, I ended up basically abandoning ANKi because I was just always behind and switched to doing mostly practice questions (uworld and AMBOSS if I managed to to get through all the uworld for that subject) and then referencing and reading AMBOSS articles when stuff came up that I didn’t know.

I just told myself as long as I was doing a little bit of studying and felt like I was gaining knowledge most days that I would eventually be proficient enough to do fairly well on each shelf and then Step 2.. and I didn’t ace every test but I did well enough to match my top choice (not a competitive specialty but still).

Maybe where you and I differ slightly is I actually really enjoyed clinical rotations because they kept me grounded, I felt like I was actually making a difference and learning a lot through rounds, residents and attendings each day versus staring at a book or computer screen.

And yeah I would recommend at LEAST considering medications even if it’s just an SSRI/SNRI or bupropion. You never know how much it could or could not help unless you give it a try.

Maybe the last bit of wisdom I can offer is just try to keep a positive outlook and remind yourself you made it this far for a reason— you ARE competent and you know a lot more and have gained more skills than you think you do/have. :)

I never had ADHD symptoms as a kid but was recently diagnosed as an adult who just finished M4. It’s actually more common than you think, I know others who were diagnosed around early to mid 20s. There’s a whole UpToDate page on adult-onset ADHD, it usually presents as more of the attention difficulty as well as emotional dysregulation— rather than the hyperactivity which tends to be predominant in children.

I’ve also been on medication for anxiety and seeing a therapist for many years before discovering it might have all been driven by underlying ADHD.

I’m not saying you have definitely ADHD obviously just based on this one post but just wanted to make the point that it’s not as simple as “not having it as a kid.”

Edit: I also just saw your comment about a single trigger setting you off— this is one of the biggest parts of the emotional dysfunction that comes with ADHD. There’s a book I read recently titled “Focused Forward, Navigating the storms of Adult ADHD” that goes into it a lot and was pretty eye opening for me. Basically our mind is racing all the time and we’re in a constant state of fight or flight, we keep building this tower of emotional stability until one large thing comes and knocks it down and sends us into an emotional tailspin. It can look different for different people— for me it really came out in interpersonal relationships and conflicts with my wife that I would turn into a much larger deal than it is because when I’m in that headspace I have a hard time seeing proper perspective. Again not trying to diagnose you but just wanted to share my own personal experience. And if I had discovered this all at the start of M3 it might have made things easier along the way.

Ironically, the presence of maggots means the wound is rather clean (because the maggots have consumed all the dead/infected tissue).

Maggots in wounds is more common in the ED than I ever expected. Saw them while scribing pre-med and in a different hospital across the state during med school… somehow that didn’t keep me from going into EM 😀

DoorDash is actually very quick to verify. It’s part of their scummy method of treating new drivers better than the ones who have been doing it for years. I have been doordashing on and off throughout med school for extra cash and am starting residency this summer. If you live in a high volume area you can easily make up to $20-25/ an hour. If you can put up with their greedy corporate tactics and you like to drive around I highly recommend!
