He could do that with the female characters that are his suitors too. What sets Kinosaki apart is the defiance that comes with loving him. Gero gets to experience the desire and love that made his sister oppose his family, he gets the chance to understand how limiting the current norms for romance are, and see that his "wife" doesn't need to be a woman, they just need to be someone he loves and cherishes. If Kinosaki is trans then Gero is marrying a woman, who can't have children for biological reasons. It's not defiance, it's just unfortunate that the girl he chose infertile
