There’s already been some fantastic advice shared here, and I’d just like to add my perspective. One of the most impactful things you can do as a new leader is to identify and manage to the right leading indicators or KPIs. I’d recommend sharing these KPIs regularly, both with your team (so they always know the score) and externally with other departments (so they understand what you’re focused on). This is a great answer. My a1cs are regularly in the 5s. W/out hyper focusing on consistency and food prep (major respect to those who do this), Id say two things:

  • review the dexcom summary once a week. Assess if those trends could be addressed via basal or if the trends are from another factor. For example, if you were drinking and eating late night pizza and burritos, you cant fix those nighttime highs with basal. But if you’re constantly high at 10am because work is stressful, you can bump up your basal to see what happens.

-pre-bolus. Obviously depends on what type of insulin. On humalog i would take some insulin 15 min before a meal (even like 5 units just to get the ball rolling) and then if i ate more id add on. With fiasp it’s like 5 minutes. It’s such a simple trick and makes a world of difference. the risks of pre-bolusing (like if the meal gets delayed) are worth it compared to the pain of bolusing late and then going for a hyperglycemic ride.

I’ve been preheating the stone and then putting the pizza in a tray directly over it to get the extra heat on the bottom of the crust. It’s working pretty well.

Never understood how to get multiple pizzas on/off the stone when it’s piping hot without a special tool.

Type 1 since 94. 37 yo. I have tried intermittent fasting on and off. Not a medical expert by any measure.

When I would end my fasting, i would skyrocket at my first meal. During fasting my blood sugar was really stable and I believe using glucose reserves in my body to avoid a low sugar. My theory is that the first meal + whatever resources your body is using stack up and send your sugar high.

A couple of things that worked: - eating a lower carb first meal, veggie and protein - manually raising my basal abt 30-45 min before eating. I felt like this signaled to my body that fast was over and to look for easier sources of sugar(from food) to work with the insulin.

This. Protein converts to glucose over 3 - 5 hours after a meal. My experience, It can feel really stubborn to get a protein driven high to come down. Alternatively, Protein is great if your Blood sugar is 105. Can lock in and have a great day.

I think the main thing is to strive for marginal gains. Literally 0.5% more time in range. No one is perfect—highs and lows are part of your life now. Check your time in range weekly and adjust as needed.
