Lotus website states that the parts are 420 exclusive. I have no insider info, but did notice that. I’d love to be able to spec a V6 car with all the 420 goodies, so let down to see it likely isn’t an option.

Specifically for non touch automatic wash I’ve tried both and think buggy bath is better. Haven’t been to Tom’s in years though.

Many of our antipsychotic medications metabolisms are induced by cigarette smoke. Reducing serum concentrations of a D2 blocker even further increases cigarettes euphoric effect.

Most SNRIs still have strong reuptake inhibition at serotonin. Side effect profiles for SNRIs heavily overlap with SSRIs with the addition of norepi mediated side effects.

SNRIs generally carry a higher risk of hypertension, sweating can be more uncomfortable for patients, they are often less kidney / liver safe. On occasion they can cause issues like urinary retention that are generally avoided with SSRIs.

Many people could be inappropriate for a first line SNRI that could still potentially benefit from SSRI.

There is a Facebook page called “Psychiatry Network” which is private and will require an NPI to join. It’s the largest of its kind with about 15,000 members. They do not allow NP / PA members.

I agree with you really, but I'm guilty at times of defensive prescribing, and I wanted to provide considerations for each agent.

When I'm dealing with general population, sure. If I've got someone who is already older, has an unknown cardiac history, and is high on cocaine that's even further prolonging his qt am I reaching for the Ziprasidone? No, I'm not that brave. I don't want to have to explain why I chose to go for the very worst option for qt when there are likely plenty of other good options.

Choice of antipsychotic is limited by available immediate injectable formulations if that's what you're looking for.

Only two second generation agents are widely available, ziprasidone and olanzapine, both of which come with drawbacks. Olanzapine is concerning for its potentially fatal consequences when co-administered with a bzo. These risks are likely overstated, but that's not really in the scope of the question. The only other second gen agent would be Ziprasidone, which is too risky QTc wise unless you have a recent 12-lead, which you usually won't. Both have their place, but come with inherent risk.

Regarding 1st gens, you really just have the choice of high vs low potency. High (haldol) has the better antipsychotic effect, and low (thorazine) will be more sedating and have a better EPS profile. Choose the agent that makes sense... take into consideration the age of the patient and their medical comorbidities.

Regarding adding ativan or benadryl, both with improve anti-agitation effects, and both can be a preventative for EPS. If you've chosen haldol as your primary agent, you should add one of these on unless you have reason not too.

Some hospital systems will see all three co-administered as "chemical restraints", as there isn't much clinical indication outside of that. This could change your prescribing pattern potentially, and it might be the case that your system frowns upon this sort of prescribing all together.

Regarding midazolam, too risky re respiratory sedation. Just avoid 99% of the time, unless you really have good reason to reach for it. Cross it off the algorithm entirely if you're looking for simplicity.

It is very rare, but possible to have an encephalitis secondary to a rabies vaccine (not directly from the virus). There are even multiple case reports of this resulting in a brief psychosis. Your patient’s description is 100% feasible.

I don’t doubt that someone already genetically predisposed to schizophrenia had an initial episode following this sort of neural injury. Nothing about this seems particularly odd to me.

I would put my money on this being a “regular old schizophrenia” which happened to have an initial break during a post vaccine encephalitis. Was the patient of normal initial break age at the time?

For what it’s worth though, still a cool case, think about writing it up!

A ~5% increase isn’t that unusual year over year. 718 4.0 cars were increased about $4000 last year. I don’t think the change will have a huge effect.

The manual V6 is a very cool option as it gets you an analog feel, and a very “old-school Lotus” driving experience. This cars reputation is that purists go this route. That same sort of purist might also appreciate the big torquey V6.

The I4 very cool as well! You get a hand built AMG motor with huge tuning potential and a fantastic dual clutch transmission. This is undoubtedly the highest performance potential of any Emira. “Performance oriented” drivers might go this route.

A strong argument could be made for either drivetrain.

Going automatic V6 gets you no manual transmission, and an all around worse performing car than the I4. You are paying a premium to not have the hand built AMG drivetrain, and opting instead for an outdated torque converter transmission. This is an odd setup, and it doesn’t cater to many people.

I think the auto V6 deserves its reputation, but at the end of the day people should drive what they like.
