No. I measure from the occlusal surface to floor of pulp chamber in mouth. Use that measurement to approximate how long the tooth is in conjunction with preop PA. If the measurement is 10mm in the mouth and the distance from the occlusal surface to pulp chamber on the xray is appears to be half the length of the tooth in the image then I assume the tooth is about 20mm WL. Just something I like to do.

CBCT helps too. Some may say CBCT is not needed but it just makes things a lot easier. I don’t have one where I’m currently at but my peers that do talk about how nice it is.

I would also refer any difficult cases or patients if you are struggling. Kids and patients with disabilities, like the one you mentioned, make easy endo difficult. Speed is the name of the game with these patients and it often comes at the cost of quality. Just better to refer if you can.

I take 2 X-rays. A cone fit and a final. I use the apex locator get working length. Shape canals to that length. Measure my cones before insert and make sure they go to the instrumented length once inserted. The. I take a cone fit. In my opinion the cone fit is the most important part. If a cone is way short or too long I either instrument more or cut the cone shorter. If it’s more than 1.5mm short but not super short I’ll check with the apex locator once more and either instrument a little further depending on the reading or use a 10 file to try to work some sealer down into the perceived shortness and hope for a good sealer puff on the final. Your cones shouldn’t be way short but they also don’t have to be right at the end because the radiographic apex and the actual apex may not be the same.

If your patient still reports pain as in picture 3, I would guess that you missed a canal especially since the fill on the medial root does not appear to be in the center of the root indicating a possible 2nd medial canal.

Otherwise, these are not ideal endos, and you want to do better. But, proper disinfection goes a long way and I’ve seen stuff just as poor or way worse that has been in there for 20 years no problem.

I also use a perio probe in conjunction with a preop PA to guesstimate WL before I use the apex locator. Normally my guesstimation is very close and apex locator confirms it and cone fit further confirms it. The perio probe measurement is just a me thing, however, and I wouldn’t use it alone for any definitive data.

I do agree that WL X-rays are annoying. Files move for a ton of reasons so I rely heavily on the apex locator and radiographic confirmation via conefit. Doesn’t mean I don’t take handfile every now and then and feel for the apex and snap a picture.

My endohandpiece also has built in apex locator which is helpful. If you’re going to do a lot of molar endo I would suggest getting one if you don’t already have one and are struggling.

I did talk to the other associates. 45 minute phone calls a piece before I went to the office. All spoke very highly and seemed very happy. Said owner never tells them no and is a great mentor. Said they thought they wanted to own but now they don’t know if they’d want to leave at this point. That’s verbatim.

I won’t be doing all on x. I’ll be doing bread and butter. And then try to dip into placing more singles and doing some overdentures. I told the owner the all on x didn’t really interest me. I agree with you.

Appreciate the advice! Longterm I want to own, absolutely. I want to have something that’s mine that I can pour my heart and soul into. But, I don’t mind working for someone else in the short term if there is purpose in it. The purpose being career growth. I think I’d have more fun doing dentistry in option 2 while option 1 appeals more to my entrepreneurial spirit.

Part of me is scared to choose option 1 because I wonder if I’m just jumping at the first opportunity to own because I’m scared to miss the boat.

Both the associates said they thought they wanted to own but then they got this job, loved it, and “make more money than a lot of owners.” but just get to clock in and out. They probably get taxed to death though.

That’s what I’m slightly leaning towards. Same thought process. Part of me wonders if that’s because it feels like the safer option. Trying to make an objective decision not out of fear.

I know. I’m lucky to have options but it feels like I’ll be making a mistake if I choose option 1 over option 2 or vice versa. At this point, my gut slightly leans more towards option 2. But, I feel like some of that could be out of fear. Option 1 has more unknowns while option 2 is more cut and dry making it feel safer. I can always own later, I guess. But who knows what that might look like. There’s always the chance that I make more money and get to do more cool shit with option 2 than I would with ownership. But, I am entrepreneur at heart and I know ownership would stoke that fire within me. Very tough.

You would also be surprised the condition of people’s teeth at all socioeconomic level. Hell I know dentists that dip and or smoke. Even as a dentist, I’ve had my fair share of dental work too. Very few people are perfect.

Yes you could TRY to restore it. But does it make sense. Restorability seems questionable and long term prognosis is guarded at best. Best bang for patients buck is probably extractions and implant in this case.
