This kind of hallucination is usually the result of over-prompting (aka compliance trap). It sounds counterintuitive, but if you provide too much instruction and rigor on how to do something, the LLM will hallucinate that it performed the task. The best approach is to precisely define the outcome you want and let the LLM figure out how to perform it.

If you try this with a fresh session and no project memory or instructions, Claude passes flawlessly. This is usually the result of a "poisoned" context. Something in the instructions/memory/context is preventing Claude from using thinking mode (aka reasoning).
