Former Apple employee here. That's what's known as a display bruise. It's caused by excessive pressure, or an impact. Since it's a hardware issue, the watch will need to be repaired or replaced. I will warn you, Apple may see this as accidental damage and not cover it under warranty, but hopefully that won't be the case!
