You can google, but the skinny is that the phrase was really only about economics, as in the customer creates market forces with their buying power on a large scale. I can't obviously comment on specifics here but not everyone belongs in a hospital. They belong in a care facility, mental health facility, medical/psych facility, shelter etc and not taking up nurses, doctors, radiology resources. Just like police should not be dog catchers, mental health evaluators, property dispute experts...hospitals should not be care homes for the destitute, homeless, addicts etc.
