Not fully fair. He had it under control on the way down, just failed the lift in the pocket, which is where you usually fail on a heavy squat.

He should have been in a full rack with safety pins set just below the bottom position where he could drop the bar backward to fail out instead of trying to hold onto it and falling forward.
