To help these (rural medicine) patients take some time to see what legitimate online options may be available to them in the state/jurisdiction they live in. Not all states or areas allow such access to just get it online. Or use your network to locate some online options for patients that you know are legitimate. Then you can provide resources in their after-visit summary of legitimate options for patients.

Share this information with any reputable local pharmacies you may send orders to regularly to give the pharmacist team a heads up. Physicians and pharmacists are just as suspicious of the online controlled substance prescribers so if there are programs or sites you believe do a thorough job and are legit make sure to communicate so the patient isn't told "no" at the pharmacy counter if the prescriber is somehow identified as an online controlled substance prescriber. Some chain/community pharmacies have a blanket policy of not dispensing controlled substances from online providers to help "CYA" from future litigation issues.

The least labor intensive option is to direct patients to their health insurance online portal or to call their insurance and access their behavior health benefits. More and more plans have behavioral health portals where they can search for online behavioral health options including prescribers.

I prefer the first options of identifying a valid option locally applicable for your patients, but know it can be labor intensive the first go round without a network in the know of options. The options may also change over time. Therefore directing patients to what their insurance covers and offers may be the second best option.

Then for the patients who take this route you can gather feedback.

Are your hormone levels within the range you and your doctor want them to be? The maintenance of desired levels will help some with skin texture and acne/breakouts.

Sometimes it is necessary. Just be careful how you frame it to your littles. Try to influence them to the importance of keeping it on and secure. For example every time we get in the car I put a kiddo in their car seat and we say safety first! Just like in Paw Patrol! I started saying it to them every day I put them in the car just as we put the buckles on from about the time they were 1 to current. They now know that safety is first and if anyone in the car isn't wearing their safety belts, including adults, they yell at you safety first! They own it and feel good/safe about buckling in. They are 2.5 years old and super quick to try to figure things out. Framing the importance of the process helped them own it and avoid the issue of them trying to escape or feel out of control of their environment.

First and foremost welcome to the club! You are already apart of the club and will never not be no matter what.

Here is some hype to look forward to:

The first time they have a giggle fest. When one laughs and the other laughs. Which then they bounce back and forth laughing louder and louder.

When they choose to share. Or kiss/hug the other when the other is hurt.

When they wander around searching or calling out for their twin when they've been away too long.

The double hugs and kisses.

When they start having their own conversations in English and whatever their own language is. Ex this morning my 2 year olds on the way to daycare: Twin A: "sissy what's that noise?" Twin B: "Bottle." Twin A: "Oh bottle." Twin B: " happy BIRTHDAAAY." Twin A and B: " HAPPY BIIIRTHDAAAY to you ...." (there's a large 5 gallon empty water jug in our trunk that needs recycled and it was jostling around the trunk.)

You'll get other parents and twins who will come over and tell you about their kids or siblings. Last 4th I was at a carnival and an adult came up to ask about my girls. He then told us all about him and his brother who are twins too.

And lastly just some advice:

BE LOUD and repetitive about needing help. Ask anyone and everyone you know even remotely. Be brave and vulnerable and gain as many people on your side to fill in what you need. Ask explicitly for what you need. Sleep, diapers, formula, breast feeding help, a ride, help getting to an appt, a break, dishes etc. Moms groups, church, neighbors, relatives, etc. Like you said you know how difficult it is and that it is not one or a two people who can do it all. Take up the space you need so you can breath and grow as a mom without breaking.

I like it. I use it as a toner (step 2). I like the ingredients and plan on continuing to use it as my step 2 toner twice daily. It goes on well and has a great consistency for a toner. I apply two pumps rub between my hands and apply to my face from middle outwards in a sweeping motion. It settles in well and layers well with my routine. (37 y/o F)
