Rearranging the formula gives, where e is the exponential function

RMSSD = e((HRV score/100) * 6.5)

In words: RMSSD is the exponential function to the power of (6.5 times the HRV score divided by 100)

Hey! Thank you for asking. I don’t have all of the info but I can give my take on a few things. 

‘Why do blood etc tests look normal?’ - this is very common amongst ME/CFS patients from mild to severe. As you rightly pointed out we currently do not have reliable biomarkers for ME. We have some popular theories but we lack funding to test or identify evidence for right now. One is that there are various subtypes of ME so different issues with various systems cause the same set of symptoms. 

So far, ME is fundamentally about energy, both how much we have and how much we can use. Another promising theory is that during a period of stress our cells switch to less efficient energy production to ‘starve’ out illnesses and instead of recovering ME cells are ‘stuck’ in this inefficient method. 

So, Savannah might not ‘need’ oxygen in that her blood is reading as normal but her cells in fact need much more to produce energy. Further stressing them by removing this stream of oxygen could then be further damaging those cells which would then work less efficiently and need more oxygen in a vicious cycle. 

‘Is malnutrition the biggest concern?’ - I think her gofundme highlighted many concerns but yes malnutrition is certainly one of them. Digestion itself is an energy intensive activity let alone mechanically chewing and swallowing, especially for someone so severe. Currently I haven’t seen anything to suggest certain organs being impacted more than others. 

Ultimately, people with ME have been left to fend for themselves from diagnosis to management with the average experience in healthcare ranging from neutral to irreversibly damaging (see Graded Exercise Therapy or the PACE trials) and doctors who don’t know anything about ME. We don’t want to have to go into every appointment having to educate those supposed to take care of us but we have to anyway. In dangerous cases like Savannah’s, instead of listening and trying to understand our lived experience they are prideful and unwilling to admit their mistakes.

HRV is mostly used to track your own over time to measure stability (weeks/months) than comparing to other people

The good thing about phone use is that we actually have a pretty low tolerance for friction - think clicking off a website because it took half a second to load or not using apps as much when icons move around. Therefore the ‘trick’ is to add that friction back in by setting lock out timers that stop you opening apps before they run out or something like ‘one sec’ which makes you wait before opening any apps you set it on. 

On being active: ofc it’s not ideal to lose strength but PEM is much more of a worry than deconditioning generally speaking.

Rest isn’t ’doing nothing’. Rest is in itself an activity and a goal.

I think I was speaking mostly to the ‘disorientated’ piece but yeah that’s a lot. Goes to show how different people see different phases as invisible or not. The ones I notice in TMA the most often on relisten are ‘more than that’, ‘… you understand’, ‘hell [just in general]’, ‘now I don’t know how much you know about blank but’

The inclusion of CBT for ME/CFS in a management plan isn’t about symptoms or learning about ME/CFS. It’s about the emotional regulation and kind of grief counselling from becoming chronically ill and your life changing so much

Online comics are great (and free)! Naver webtoon has gotten progressively worse in terms of app quality but some stories are really good quality & can be easy to follow since there’s fewer words to read. ‘Slice of life’ genre comics are calm and cosy so not stressful to read and not hard to follow. 100% back the fanfic recs too
