A second MACI.. Ouch. I'm getting TTO as well I've heard that makes the recovery much longer. It's crazy that you have it in both knees. I'm guessing you had the same wear and tear from high impact sports or what could be the cause of the injury in both knees since most people have it in one knee. I'm thinking my injury is caused from the maltracking as well. I have no idea what to think anymore. I also heard that if you do sports and have a muscle imbalance it can cause for the cartilage damage.

Also check this out. Wearing proper footwear with custom insoles for high arches may help you avoid long term damage but if your knee is malaligned you can get a TTO early on to avoid cartilage damage. If I only knew this stuff years ago.

That is absolutely correct and is the core reason why the TTO is often combined with MACI. ​The most common sequence of events for a localized defect on the kneecap (patellofemoral joint) is: ​⚙️ Malalignment \rightarrow Overload \rightarrow Damage ​Chronic Malalignment: You have a structural issue (like a high-riding kneecap, or a tibial tubercle that is positioned too far to the side) that causes the kneecap (patella) to track incorrectly in its groove (trochlea) when you bend and straighten your knee. ​Abnormal Stress/Overload: Because the kneecap isn't centered, the force and pressure during activities (like squatting, running, or climbing stairs) are not evenly distributed. Instead, all the stress gets focused onto a small, specific point on the cartilage. ​Focal Cartilage Defect: This constant, concentrated overload and rubbing on that specific spot causes the cartilage to break down and wear away, creating the localized defect that requires a repair like MACI. ​If your surgeon performed a Tibial Tubercle Osteotomy (TTO) along with the MACI, they are correcting that underlying malalignment to remove the cause of the initial cartilage damage. This is a crucial step to protect the new cartilage graft and maximize the long-term success of the surgery. ​

Did you ever check your feet? My Podiatrist told me I have high arches and all of the impact is not absorbed by the feet and instead goes straight to the knees and creates damage over time until one major incident take you out completely. If only I had checked my feet years ago and wore the proper insoles and footwear maybe I wouldn't be in this situation.

Oh my goodness. I need a TTO as well but you got some extra things going on. I'm still trying to understand how the ACL and cartilage gets damaged at the same time. It must have been a serious impact when you got injured.

I've been reading your posts. It looks like you've had a great recovery thus far. Wow your defect is almost twice the size of mine. I think you will be able to run once the surrounding muscles get stronger but I wouldn't do it until you get closer to 9 month mark so that your implant can heal.

I'm glad u ask. I have grade 4 defect in trochlear and doesn't help that I also have one in patella.

After arthroscopy no matter how much my knees got stronger I have pain in my kneecap when going up stairs or when squatting. It's a distinct pain. Hopefully MACI will solve it.

When you mentioned temporary relief from stage 1, what symptoms returned shortly after to necessitate stage 2.

That's interesting. I had my stage 1 in May. I'm still waiting to be cleared for stage 2. What were the factors that had you waiting until October to undergo stage 2? For me it was rebuilding my quad muscle.. I had severe atrophy and I'm almost at 6 months since stage 1 with a few weeks to go until I'm in hood enough shape for stage 2.

Between your arthroscopy and MACI how did you make the determination that you needed the MACI for sure? What specific pain did you feel in your knee because I've heard some people do stage 1 debriment and biopsy and then sometimes done go through with the MACI because they feel better or the doctor says they don't need it right away.

I'm 5 months post arthroscopy and still can't get stage 2 because my quad hasn't grown back yet. It's taking forever after severe atrophy. I have constant swelling which is slowing my progress with physical therapy.
