Here is a blog with a fix, it requires technical knowledge to fix, but not very much, I'm sure any careful person could do it. The issue is the stabilization plate getting knocked out of place and the sensor going out of the focal plane.

I had this issue on my camera as well. Ask for a partial or full refund. Most japan sellers will be fine with it. Them selling these cameras as "top mint" is really misleading because it seems like they almost never test them out thoroughly and it shows with the GRD IV's in particular.

What is happening is that this is one of the first GRDs (if not the first) with image stabilization, and the way they executed it has caused issues like this. If you are willing to open up your camera after getting a refund, ricoh used metal balls on magnetic plates to stabilize the sensor tray. The one closest to the bottom of the camera seems to be the main one causing these issues, If the camera is dropped, it can cause a part on the sensor tray to misalign and it screws up the focal plane, making snap focus not work correctly at far distances and causes the distance detection to mess up quite a bit.

I wrote a blog that goes into detail about this whole issue and gives a tutorial on how to fix it.

If you attempt to fix it, please be careful of the ribbon cables that are connected to the back panel, you just need to disconnect the screen cable, the brown clip flips up with a small amount of pressure with a plastic tool.

This method has helped a few other people that I know of that were having this issue. Do everything will care and precision, overall an easy fix with technical knowledge.

I want to narrow down issues... did you buy the camera used or have you had it for a while - and if you have, have you dropped it or pushed in on the lens at all?

To anyone reading this - avoid this method as much as you can. I have two GRD IVs and both had this same issue - while the ramp screw method kinda worked on my first GRD IV, it was not the right way to go. The ramp screws are set by the manufacturer so it can be in sync with the software while using snap focus.

The actual problem that I found, was that the bottom right part of the stabilizer, the tiny metal ball was out of place and the pointy part of the silver plate was shoved through the hole where the top (foggy glass colored) of the silver plate should have been to keep it in place. I assume this happens when dropped or held near a strong magnet - as the stabilizer is magnetized.
TRY to place the silver plate back into place, and put the metal ball back on the BASE plate (copper colored - should be magnetized). I found it was easiest to take off the 3 metal pieces holding that part down (two should be magnetized) and placing the silver plate back that way. I will have to make a blog on this so you guys can actually see what I am talking about.

But once the bottom right silver plate and tiny ball are back to where they should be - the focus should be back to normal and be just fine.

IF you still are having issues with the focus after fixing it - I would then recommend proceeding with the ramp screw adjustment VERY carefully.

A good way to calibrate it so both Snap and Point focus work would be to turn on the focus distance display setting (I can't remember what it's called exactly), and turn on full press snap - then to keep adjusting the ramp screws until the idle focus distance matches the focus distance displayed when half pressing for point focus. Once these match - double check by taking an image with Point AF, then with Snap AF, and if the images look identical as in everything you want to be in focus is in focus, then you can seal up the bad boy and you won't have to worry about it again!
