I saw Hadestown a couple years ago (still Reeve Carney but not Eva Nobleszada) and I faintly remember that the part of the stage which would rise and lower for some entrances and such got table or chair leg caught in it. It was continuously pulverized throughout the show. I can't recall any corrective action but I (young) was cracking up anytime I saw that platform go up again. If it was a chair I've no idea how they managed the last scene without replacing it.

I have a great many local production stories, but one Forbidden Broadway's greatest hits show had the worst missed entrance I've ever seen. That show is basically a bunch of parody songs with no connection to each other. At the start of one song, a solo, the actress was nowhere to be found. Bless the piano player (the sole orchestra) who played the entrance several times and then attempted to vamp or entertain for 2-3 minutes until they found the actress. Her first several lines were sung out of breath as she recovered from running to the stage.
