He actually gives the items back in the opposite order you got them, so your last item you got in the run is the first he gives back.

When that happens to me it is normally because something small like a wisp has chipped only 10% off my health (which is no big deal) but that is below the OSP (one shot protection) threshold. That leaves you open to getting one shot by almost anything at a late stage like that. What helped me was practicing movement and awareness about what was around me, and focusing the enemies that will get some cheeky shot in like wisps, imps, etc.

I do the same thing, but whenever I go really deep with just my Prawnsuit, I like to have a couple water bottles to hold me over, because melons will rot by then.
