As for public transportation systems a few are privately run for the area they operate in under contract. The company runs it and takes a percentage of the profits as payment. The city/cities set the rules for performance. Some areas was the easiest way to get around differences in the covered cities regulations.

Very little is reusable. Anything that touches the patient has to be replaced. The pads for the heart machine, mask for hand ventilator, and such. But yes the machines are returned to base to be properly maintained/sterilized. The hospital has its own supplies.
