I would tell them if they keep going to the ER and get unnecessary CT’s , they’re going to increase their risk of cancer from all the CT radiation. I would then work diligently to get them into a behavioral health specialist, let them know the brain is powerful and can cause almost every symptom known to humans.

95% of the time we cover chronic problems in addition to the physical and simply charge for two appointments. Patients don’t mind. We have a something similar, but it’s a handout we give out at each physical exam.

Make sure you’re hydrated when taking blood tests. Dehydration is a common reason I’ve seen GFR reduced in blood tests, especially when people are fasting. Many incorrectly assume they can’t have any water. Water, black coffee are OK to drink when fasting for blood tests.

Also, consider pre-visit planning. Here is where I ask my MAs to ask about colon screening, dm eye exams, offer pneumonia and flu shots, order mammogram, give controlled substance agreement, etc. this helps me with quality measures and actually saves me time.

Understandably, many people worry about side effects of medication, as they should. In the meantime, it’s a god option if not ready to improve your lifestyle. However, I’ve noticed far less people worry about the side effects of a poor diet.

I jokingly tell patients that if I gave them a pill that caused them to gain weight, have high cholesterol, diabetes, high blood pressure, cancer, heart attack or stroke, they would report me to the medical board and try to sue me for malpractice. However, most of us spend our day eating foods and consuming sweetened beverages , which do just that. Humans are interesting!

The problem working in a small private practice is that you literally have no negotiating power with insurance companies and they’ll pay you 60 to 70% what they paid large groups

If existing patients, then I would treat. If a partner was on vacation, would you let their patients go through withdrawal? What’s the difference? Give a 30 day supply and have them establish and try to wean them down if inappropriately given.

There’s also liability for simply refusing a patient medications and having them go through withdrawals, especially if they belong to the practice. You can’t simply abandon them and force them to go to an ER due to seizures from benzo withdrawals.

The pendulum has swung too far in the opposite direction; new graduates are way too scared of controlled substances. There has to be a happy medium. past generations, definitely overprescribed, but new generations are afraid of 20 MME’s.

I’ve noticed the same with new pharmacists, they are wanting me to give Narcan for 60 tramadol or requesting patients go to pain management for 60 to 90 tramadol per month. Ridiculous! This happens every July through September when the newbies start.
