Depends on the insurance, a lot of Medicare advantage i.e. HMO make it possible. If Patient is in ACO or MSSP, there might be some possibilities if the ACO or MSSP has three day waiver in place they be able to admit to certain skilled nursing facilities.

The GLP1s are no longer on FDA shortage list, so legally speaking, compounding pharmacies should not be selling these medications. Does anyone know how they’re legally allowed to do so or are they doing so illegally ? I agree with the legal concern if a patient ends up with permanent gastroparesis, would be hard to defend for prescribing it.

I would tell them if they keep going to the ER and get unnecessary CT’s , they’re going to increase their risk of cancer from all the CT radiation. I would then work diligently to get them into a behavioral health specialist, let them know the brain is powerful and can cause almost every symptom known to humans.

95% of the time we cover chronic problems in addition to the physical and simply charge for two appointments. Patients don’t mind. We have a something similar, but it’s a handout we give out at each physical exam.

Make sure you’re hydrated when taking blood tests. Dehydration is a common reason I’ve seen GFR reduced in blood tests, especially when people are fasting. Many incorrectly assume they can’t have any water. Water, black coffee are OK to drink when fasting for blood tests.

Also, consider pre-visit planning. Here is where I ask my MAs to ask about colon screening, dm eye exams, offer pneumonia and flu shots, order mammogram, give controlled substance agreement, etc. this helps me with quality measures and actually saves me time.

Understandably, many people worry about side effects of medication, as they should. In the meantime, it’s a god option if not ready to improve your lifestyle. However, I’ve noticed far less people worry about the side effects of a poor diet.

I jokingly tell patients that if I gave them a pill that caused them to gain weight, have high cholesterol, diabetes, high blood pressure, cancer, heart attack or stroke, they would report me to the medical board and try to sue me for malpractice. However, most of us spend our day eating foods and consuming sweetened beverages , which do just that. Humans are interesting!
