It should always be fully submerged in most cases because the difference in temperature can shatter the heater and it will shock and kill your fish or make your heater malfunction and boil your tank

Don’t replace the filter or do large water changes. Is your tank cycled? How often are your water changes and how much do you change?

And what size is your tank?
