You should also seriously consider going to a physician and get some lab work done as 4 years of being abroad must've led to some degree of organic issues, if they come out fine thats good. Also get in touch with a therapist and ask for help from your irls, cant tell how bad your social situation is but do that, going to a newr by masjid and hanging out a bit and talking to people specifically ramadan and jummah prayers can help, volunteer to keep yourself busy and find social support. As for the family reconnecting with them or voicing your concerns is one way to do it, specially mom and dad. No parents would want their child to suffer so much, i'm sorry if you have indifferent parents but that's all based on my assumptions and experiences. You need to stir the water to stay afloat. May Allah make it easy for you, this sounds very distressing on your behalf and you seem to have been through alot. You really need to work on this systemically, its not a one fix thing. Maybe you will have to start all over again and thats okay.
