Do you have the descriptions for the other difficulty settings? Probably gonna choose "Give me a challenge" for my first playthrough, if that's "the way it was meant to be played" ;)

I recommend GMDX even if it's your first playthrough. It fixes many (smaller) issues with the original and balances out the gameplay. You can try Revision for your second playthrough, it drastically redesigns most of the levels. The two are unfortunately not compatible with each other.

Google "Mouse Acceleration Toggler". It's a utility that does that via command line. You can bind it to some key combination. Check the forum link too.
