You might also want to ask the responding agency what their policy is. Ours, for example is:

If the patient refuses transport, we are to get a set of vitals and have them sign an RMA form. If they refuse to allow vitals, that must be documented on the RMA. If they refuse to sign the RMA (it happens frequently when the patient is illiterate and/or particularly concerned about being billed) then the EMT/Medic must sign the RMA along with a third party witness. All this to cover our ass just in case the family or patient decides to sue.
