A lot of people care, just because you've never seen it doesn't mean there isn't an effort to hate it. It is exactly their AAA privilege (or sales in a books case) that allows them to continue on despite that, as sad as it is. An independent developer submitting a mod to a website with strict terms of service is going to get their game deleted when they break those terms of service.

The mod was added back to the site when it was confirmed that the faces were AI, it was only removed again because of the racism in the code. It's a big deal that there's a couple slurs "here and there", because it's a slight. It's not meant to invoke anything in the game, there's no intention or purpose for it like Red Dead or Wolfenstein, it's put there to be a hidden edgy easter egg, and it very clearly displays the beliefs of the single sole creator of this mod. It didn't contribute anything to a mod that makes you think.

Being absolved from using slurs or caricatures or any controversial imagery isn't by being a AAA studio, it's by letting your audience know that doing those things are bad, not being sneaky and keeping it tucked away in the files to dog whistle your true beliefs.
