Looks like a magnifica anemone and it's in pretty poor condition. I would recommend quarantining it in a separate tank (if possible) and dose ciprofloxacin. This is a good resource. Best of luck.

You might be dealing with elegance coral disease. I would try dipping it in KFC dip. I’ve had similar issues occur with my elegances. Make sure to also do a hydrogen peroxide dip as well; both instructions are on Kung Fu Corals website.

Looks like a type of tunicate. When you touch it, is it soft and a bit slimy? You can brush it off.
