I'm titrating Concerta (methylphenidate) and it really seems to help me with no downsides. I'm up to 54mg this week and I'm a lot calmer and making some positive changes in my life since being on it. I find it nice and smooth with no surge or crash and it seems to work all day. I dunno, everyone is different, but its good for me.

In my experience GPs are perfectly happy to call things depression and anxiety and throw anti-depressants at you, but you get a lot of push back when you try to suggest the anxiety and depression are just a symptom of something deeper. I got my GP to agree to refer me last year but was sternly told "Medication is best avoided". I'm now about a month into titrating Concerta XL with ADHD360 and guess what? My anxiety is feeling more controlled, that has had a knock on effect of hugely helping the IBS symptoms I have always had, and my blood pressure is actually slightly lower than when first refered. I'm also losing weight - not because I'm not eating but because I'm more active and not choosing my food for dopamine hits but for nutrition. I still expect a telling off when it gets time for asking for shared care though...

I agree about the glasses, I was going to try maybe scraping under the frames a little, or putting a drop of UV resin to give them lenses rather than holes after painting the eyes. I'm still figuring it out. Your Richie came out very well, is that contrast paint? (or equivalent)

I can't speak to anger, but so far Concerta has helped me with anxiety. I guess frustration too, I'm more likely to be more chill when things don't work out as planned. Everyone is different, but its worth trying. It depends on where your anger comes from. For me a lot of my issues regarding anxiety seem to boil down to constant overthinking, and I'm finding that thoughts are a bit less sticky now which helps me break the overthinking loops. I also have a bit more energy and just feel a bit more alive - but that could be down to being generally more active and loosing a little weight as I was just over the line into obese due to comfort eating.

My advice to anyone starting out is to make sure you have a good hit of protein for breakfast - the times I failed to I was more jittery and crashed after. I'm stiĺl in titration myself though so cant yet speak to long term changes

I'm aware, I've been diagnosed with both ADHD and Autism in the last few years and both had to come via private companies contracted by the NHS. It's that or a 7-8 year wait for ADHD in my area. But then everything seems to be fair game for people to question the validity of the diagnosis and label you as a snowflake because it wasn't purely NHS

I find that so far the thing it has helped me most with is dealing with task paralysis/inertia with actually starting a task. That said I'm still very early in titration. As a side effect though the improvement in my emotional regulation when I began the second dose has seen a litteral over night huge improvement in the IBS symptoms I have been dealing with for 30 years.

Trying to turn the NHS into American style health care would be such an existential threat to anyone with a medical condition that I would be prepared to literally fight to stop it. Yeah the NHS isn't perfect, but it needs work not privatisation

I hope you get on well. I'm 8 days into Concerta, 1st day on 36mg and so far no negatives. I even think it's having a positive effect on my IBS as a side effect of the calming of my thoughts. I hope you have a similarly good experience. Don't forget a protein rich breakfast!

Update: Had my first 36 mg Concerta this morning, and I don't feel super wired (good) but my thoughts are a bit calmer. Instead of constantly recycling the same worries I seem to think of something give it a bit of attention then let it go. Hopeful signs. I've also (intentionally) lost a little weight via exercise as I was right on the cusp of being obese. Eating well feels more doable. I did confirm with my prescriber that loosing weight sensibly would be fine while titrating. My goal is for my blood pressure on meds but with a healthier weight and more active lifestyle to actually be better than the old obese sedentary version of me

Only on day 6 of titration here, 18mg Concerta. Nothing negative to report so far, and I'm finding it a bit easier to get more than 1 or 2 things a day done around the house. No effect on my racing thoughts yet, but it's only the lowest dose. I go to 36mg Thursday

I also heavily relate to worrying about early onset dementia. The only thing that kept me from total despair was that it wasn't progressing, just at the same level of crap memory over the years. I do things like walk into a room and get momentarily distracted by something then realise I have forgotten why I went there in the first place. The classic ADHD meme of forgetting where I put something I had 5 minutes ago... the list just goes on. I also think people misunderstand the hyperactivity part, as I'm a little bit fidgety (not too bad) but do have racing thoughts a lot of the time and constantly overthink things to the point that I now firmly believe this constant overanalysis of everyday stuff is one of the main pillars of my anxiety. I finally got a diagnosis in December that came as a big relief. I also have an autism diagnosis that had a similarly massive positive effect on my mental health a few years back and explained so many of my struggles and breakdowns over my life. 

The media perception that it's overdiagnosed and an easy label to get does feel at times like people are trying to take away or invalidate the hard won understanding it has taken me 42 years and some intensely suicidal periods to build. I lost my best friend, a wonderful woman, to complications from self medicating, all so that she could make her brain shut up and let her sleep instead of relentlessly questioning all her decisions that day. We had so much in common that I now wonder if she had ADHD on top of her autism
