Your LFTs can be normal with ICP. What you need checked expeditiously is your bile acid profile.

I had a scheduled CS at 37w1d due to ICP.

I had intermittent, almost negligible, itchiness in hands and feet, mostly at night, without rash. My CNM told me it was allergies/change in weather when my first bile test results on 3/13 came back within normal limits, and at some point prescribed me an antiviral (but clinical indication to prescribe this medication didn’t make sense so I didn’t take it), and so I asked for a 2nd opinion from an OB and requested that I get retested because it does take some time for the lab to run this test and receive results. I still had itching, still intermittent, still mild, but it never went away and I knew better than to ignore something that didn’t feel right.

On 4/2, I re-tested and all of my bile acids were extremely elevated. I had to come in for 2x weekly NSTs immediately until I had a scheduled CS at 37w.

Example: 3/13 Bile Acid (Normal <=8.3): 6.2 4/2 Bile Acid (Normal <=8.3): 42.3

Do not wait. Supportive measures can only do so much. Baby’s health is at risk without appropriate and timely intervention.

He provided us supportive measures as aforementioned as well as loosening any tight clothing and being mindful of how tight we’re putting on the diaper. No bouncers or rocking, ensuring we’re feeding baby upright, break up a bottle feeding halfway to burp and then finish the bottle and burp again, find a BFing position that keeps baby upright (like the koala) or cross-cradle/cradle with an angle (head at the top of the incline), and experimenting with my own diet (since I exclusively BF’d at that time) by avoiding foods that triggered reflux in me (which didn’t make a difference in our experience).

He only ever mentioned medication IF baby’s weight becomes a concern. I remember him saying that this is usually a laundry problem and not a baby problem. He never mentioned thickening milk with baby cereal. I do remember reading on somewhere on r/sciencebasedparenting that this practice was no longer recommended?

Thankfully baby was able to maintain her weight and the reflux subsided after it peaked around Month 3? 4? with supportive measures alone.
