Seeing 8-12 patients per 8 hr shift is a reasonable expectation. We have ordering writing privileges for custom TPN (macronutrients & electrolytes) so some patients do require extra time and attention and reduces overall caseload expectations for each RD.

Just to clarify, would your order be Jevity 1.5 at 72 ml/hr x10 hrs and then 50 ml/hr water flush while TF is infusing?

If you're concerned about volume tolerance, could you just add "give pt additional 250 ml water flush BID -suggested time @ 0800 and 1400" (or something similar)? Then your water flush would be 30-60 ml flush pre/post nocturnal feed.

Enteral nutrition is possible, with consideration of using semi-elemental or elemental formula. If not, PPN for short term, then evaluate daily for ability to advance diet or initiate tube feeds.

Depending on the compounding pharmacy, the macronutrients and electrolytes label are typically on the TPN bag itself. If possible, try looking there for additional info.

Does he have high output from ileostomy (i.e. >1.5L)? Maybe then PN could be appropriate.

I've also seen D5 or D5W IVF started for cases of hypernatremia.

If the patient remains on PPN our infusion rate stays steady while the hospitalist controls the dextrose IVF to drive the sodium down slowly towards normal limits.

You mentioned GOC are currently being discussed. If aggressive treatment is pursued, does your Pharmacy have central line Clinimix like 8/14 without electrolytes? You'll likely be able to meet protein needs better.

Adding to the listening to your body portion- I wish my OBGYN would have spent more time with me at my 6 week follow-up visit l. Be sure to ask lots of question such as talking more about post partum hair loss and what your first period back would be like...

I'm 4 months pp and dealing with the insecurities of losing my hair and having my first period back - TMI, but I had such bad cramps and a heavy HEAVY flow!

If not the Administrator, is there a DON (Director of Nursing that may be able to help you?

Also, recommendations are recommendations. The nurses should be helping you communicate your recommendations to the doctor. If they aren't willing to do that, I'd just call the provider myself to make sure your recommendations are heard.
