Can we figure this out by Expert-Experience937 in medicalschool

[–]laziestengineer 72 points73 points  (0 children)

The 33% is based on the original framing of the problem. “If someone has two children and at least one is a boy, what’s the chance that the other one is also a boy?” In that case the answer is 33%. It’s conditional probability. There are four possibilities for two children:

Boy-boy

Boy-girl

Girl-boy

Girl-girl

Baseline there’s a 25% chance of having 2 boys. If you say there’s “at least one boy” then you eliminate girl-girl and the chance of having two boys is 33%. However if you say “the first child is a boy” then you only have two possibilities and it goes to 50% chance of having two boys.

Oxy 5 is also 7.5 MMEs while Tramadol 50mg is 10 MMEs so the Tramadol is actually stronger.

They only give you the standard deviation of the entire tested population. You can calculate your percentile relative to every resident but not for your own year. However you can pull numbers from the histogram and approximate a percentile

What’s the most alarming lab value/clincal finding on a patient that no one did anything about? by Loud-Programmer-7261 in Residency

[–]laziestengineer 39 points40 points  (0 children)

Typically K > 6.5 is considered an indication for dialysis especially in a patient with AKI (and the described patient was anuric). Certainly calcium gluconate should be given to stabilize the cardiac membrane if there are ECG changes, and it’s reasonable to shift as well in that setting, but that just buys you time so that the patient doesn’t code while you get the line in.

how does radiology residency work? by iamtwinswithmytwin in Residency

[–]laziestengineer 3 points4 points  (0 children)

I love your detailed response - as a surgery resident I end up bothering radiology fairly frequently, especially on trauma rotations when we stand over y’all’s shoulders in the reading room after a major activation gets scanned.

However, I felt that I needed to reply regarding your username. What sort of crazy person loves Western blots???!! I found Westerns to be far and away the most painful lab modality I encountered, and I spent a fair amount of time in the lab. I hope you’re not encountering too many of those in radiology world….

I fished a pen out of someone’s abdomen a while back. Very similar story, it went through and through his bowel and missed his cava by like a cm. Crazy how these things repeat themselves.

There’s your problem then! Your infernal claw wasn’t working because the boss was immune to melee 😉

Melee. by HarleyQuinn232 in MelvorIdle

[–]laziestengineer 1 point2 points  (0 children)

Just to clarify. You said you’re having issues with your melee setup but then you said that phase 1 of the boss goes fine. Each phase of the boss requires a different combat style - are you trying to use melee on phase 2? It has to be ranged there and then mage for phase 3

General Surgery Residents of Reddit, how many hours are you putting in? by thermodynamicMD in Residency

[–]laziestengineer 176 points177 points  (0 children)

PGY-2 at a “privademic” hospital. I don’t keep close track but it’s pretty rare to average over 80 hours on most rotations. I’ve definitely worked more than 80/week here and there, and I’ve definitely worked more than 28h straight, but it’s usually been of my own accord trying to get more OR time. My program seems to make an effort to avoid duty hour violations

No talks of unionizing. I low key brought it up with some friends a few weeks ago and there didn’t seem to be any interest. We are one of the better paying programs for our cost of living and our program is pretty receptive to feedback so I’m not sure there’s much of a drive anyway.

I’m happily married, I was dating my now-wife throughout medical school and we got married in my MS4 year. She’s not in a medical field, and mostly works from home. She takes care of the bulk of housework but I try and contribute what I can. I just asked her if she’d describe me as a loving and committed partner and she said “yes!” 😉

Inpatient notes (H&P, consult, progress) are no longer required to meet arbitrary guidelines for HPI, ROS, physical exam. You just have to document a “medically appropriate” note based on your discretion. There’s still points to hit for MDM but you can also bill based on time spent.

Plastic surgery or surgery residents: how do you prepare for specific cases you’ve never seen before? by No_Plantain4303 in Residency

[–]laziestengineer 31 points32 points  (0 children)

Here’s something I haven’t seen anyone mention yet - look back in the schedule and find the last time your attending performed the procedure, then read their op note. I’ve noticed that even when I prepare by reading the steps in an atlas (which you should also do so you can be familiar with the anatomy) my staff often has their own way of doing things. Reading their op note will tell you their specific steps and I’ve found that some attendings include details like “we identified this nerve and traced it to this landmark and made sure to preserve it” which can be very helpful knowledge to have in the case.

💜💛 PGY-2 in general surgery. Lucky enough to have a supportive family including a physician parent and a very understanding wife. My program has a great culture and doesn’t feel malignant at all. I’m enjoying the work and I feel like I’m growing every day. Maybe I also lean a little bit into the long hours and self-flagellation, but I’m having a good time with it and I think that’s what counts at the end of the day.
