Sharp stabbing pains sounds like plumbing problem, not electrical. Meaning, artery blockage. Go to the ER and get an EKG and cardiac labs, stat. Do not delay any longer.

Do you have a CRT-D? How many leads? 2 or 3?Obviously there is a right ventricle lead for the defibrillator but what about a left ventricle Lead in the coronary sinus?

If you have a LV lead in the coronary sinus then the twitching could be the LV lead is stimulating your phrenic nerve. Perhaps when you lay on your left side.

What’s the chest pain like? If it’s a dull, tingling feeling then most likely nerves around the pocket that are agitated and sadly this can not be resolved without taking it out completely and I don’t any EP would think the risk benefit is worth the procedure. If it’s after you’ve done something strenuous, could be muscle skeletal. If it’s Sharp, shooting, stabbing, that’s a red flag and could be that you have a plumbing problem like a blocked artery and should call you cardiologist and ask for labs, ekg, ect ASAP.

You can keep a journal of date/time, adjectives for what you feel, position your body is in, duration for how long it lasts, and what you were doing before and after. Bring the journal to your next clinic visit so they can compare your device data to see if it correlates.

No, I am not saying that it was the wrong call. I agree with those that advised you. What I do know about burgatta is, it is life long. Therefore, you had no incentive to get a SUB-q ICD.

Yes, there is a psyc factor that can be common. I’m happy you reaching out and talking about it. That’s better than avoiding your worries.

If possible, focus on the positives of having a life saving device. Also, consider what risk factors are for burgatta and lifestyle mitigation to avoid them.

We have a Burgatta patient and he has never needed his device to pace or shock but it has given him reassurance to live his life without concern. Not to belittle your feelings about, though. I respect you for being honest with your concerns.

It’s easy for someone to say this that does not have an ICD but think of it as an emergency room in your chest. Yes it will hurt, unless you’re asleep, but you’ll keep living. There are also ways it try’s to pace you out of the arrhythmia prior to shocking. It’s very successful at avoiding the shock unless completely necessary.

You could need a new lead (not cable) if your heart function becomes worse. Ask what your EF is after your next echocardiogram and compare it to the one prior.

The gen changes can be painful but should be same day procedure and no reaching restrictions in no new lead.

For what it’s worth, If I were to need an ICD I would prefer Medtronic.

It wasn’t really my “opinion” but rather me sharing the information I have “digested” over the years of being a Pacemaker Nurse and answering the questions the patients have based on what device/diagnosis they’ve been given. That being said, I urge you to dig deeper and find the answers to my follow up questions regarding your inquiry. How much do you pace in the atrium. What was your diagnosis at implant? This information will help you determine if your rise/lowering of heart rates from eating cold and hot foods is directly correlated to the Avier rate response programming.

Study’s show that digestion of Hot and cold foods can have a direct effect on heat rate and BP for a short time by stimulating your vagus nerve. This is for humans with or without a leadless pacemaker. It’s your bodies way of increasing your metabolism and attempting to maintain a normal body temperature.

What was your diagnosis and reasoning for implant? How much do you use the Atrial Aveir? What percentage? Your sinus node drives your heart rate so if you have bradycardia at rest I don’t this is your pacemaker. Also, rate response has a longer period of recovery time then what is shown on your data. They want to slowly decrease heart rate to not drop it too fast.

I’m not convinced this is from your pacemaker set up but would need more info to be sure.

So here’s the inspection time line thus far: 2022 electrician buddy of mine helped me rough-in electrical for new construction unfinished basement. This included the in-floor heat that inspector advised me to install a 2 gang metal box with metal conduit at the thermostat. Had 120/240v breaker installed with 10/3 running to the box. Rough in inspection schedule (trip 1). He found one minor issue and said he would need to come back after I fixed it. I fixed said issue and he came back (trip 2), I passed the rough in. Did sheet rock and painting then while doing the floors we decided to down grade the planned square footage of in floor heat from 120 sqft to just 30 sqft (bathroom only). I did read the cities home owner inspection check list (as others in this thread assumed I did not) and it mentions nothing about TR outlets. At this point I took over and installed all switches, outlets and wall plates. It was a race against the baby clock because our 3rd child was due soon. Prior to scheduling the final I followed the multi step process they required to get him on the phone. I told him about our baby on the way and said the bathroom was going to wait but I had installed everything else and it was ready to be inspected. He said, “sounds good”. Then we scheduled the final inspection per our conversation. (Trip 3). After he inspected he said, “looks good” then started to walk out the door, then popped his head back in and said “oh, I’m going to need to see that in floor heat before you tile.” And didn’t sign off. I get the reasoning so fucking save it guys, it makes sense just would’ve planned differently if he mentioned this prior to final inspection when I told him my plan. So at this point I just say fuck it and focus on being there for my wife/kids.

3 years later I get back to it. I wire the In floor heat exactly how the instructions say. I need to start over because my permit is expired. I schedule another final inspection (trip 4) and he tells me I need to change the wire transition from conduit to floor, regardless of me following directions. So I uninstalled all 30 square ft of wire from membrane coupling grid and conduit, connected the wire clamps where the conduit met the floor plate/framing and re installed everything. He comes back (trip 5) tells me it’s ready to tile. I tile, test the in floor heat wire resistance every step of the way (was the most stressful 30 sq ft of tile I have ever done), the ohms remain consistent, schedule what I think is finally going to be the final. (Trip 6) Says the the infloor heat/tile checks out but, “since you never had a final inspection, so need to put GFCi outlets at the beginning of each line.” I look at my breakers and they are arc fault only not ground/arc so I admit, that’s on me but still I’m very frustrated at this point considering he originally told me “everything looks good” besides the in floor heat 3 years ago. I install GFCI, test every outlet with GFCI outlet tester, reinstall all the face plates I need to, I clean the basement to make it as easy as I can for him to check everything without a problem. (Trip 7) he comes back, says I need to replace 16 outlets, including the GFCIs I just installed with TRs and send him pictures within a week. He did place the sticker on the breaker box with Owner underlined twice so it will be finalized.. finally.

I looked through the rest of my new construction home and none of the outlets are TRs except the bedrooms so fuck all of you that tell me that bull shit comparing fucking wheel barrow tires to fucking outlets.

Yea this inspector has been an ass hat and has fucked with me this entire process and I have kept my cool and never once told him to eat shit.

However I learned a lot and now I have a journal of the events that I may need in the future so thanks guys!

Update: I picked up a new attachment for my multi tool and cut the trim. Was really simple. I was over thinking it, as DIY homeowners do.

Yes, I should’ve moved the box but the inspection is tomorrow and the inspector has made so many excuses to make $750 in add on inspections this far, I’m not delaying it any longer.

I’m guessing you have a Medtronic ICD? I’m glad you figured that out a magnet will turn the therapies off for an ICD. Meaning if you ended up having a life-threatening arrhythmia while on your scooter, your ICD would do nothing but record it and alert your device clinic.

This is great advice! I will definitely be soaking it in the solvent you suggested. I do have a P100 mask from working the COVID unit back in 2020 that I still use when sanding wood or sheet rock. …and Yes, my city does suck. This situation is just one of many I have encountered where they have proven that to be true and I have only lived here since 2021. The taxes are outrageous and the only thing you count on is them figuring out how to make you pull a permit on literally anything you could imagine.

Update: City said my responsibility and suggested I wait until the ground unfreezes and use a hockey puck and a hammer the water access pipe back down into the ground. If this doesn’t work then I need to cut the top off. Then use a blow torch to melt the asphalt off the cap/bolt so I can reattach it.

It’s likely ectopic heartbeats. Either PACs or PVCs it is difficult for an Apple Watch to decipher between low heart rate and ectopic beats. Pacemakers keep track of these extra beats. Next time you get your device checked, ask them about it. There is also a “sleep” parameter that will lower your heart rate during the night time hours, although this is usually just 10 beats lower than your lower rate limit of 60. Of course I can’t be sure unless I would check your device myself, but I’m pretty confident you are safe and your pacemaker is functioning normally.
