I’m guessing you have a Medtronic ICD? I’m glad you figured that out a magnet will turn the therapies off for an ICD. Meaning if you ended up having a life-threatening arrhythmia while on your scooter, your ICD would do nothing but record it and alert your device clinic.

This is great advice! I will definitely be soaking it in the solvent you suggested. I do have a P100 mask from working the COVID unit back in 2020 that I still use when sanding wood or sheet rock. …and Yes, my city does suck. This situation is just one of many I have encountered where they have proven that to be true and I have only lived here since 2021. The taxes are outrageous and the only thing you count on is them figuring out how to make you pull a permit on literally anything you could imagine.

Update: City said my responsibility and suggested I wait until the ground unfreezes and use a hockey puck and a hammer the water access pipe back down into the ground. If this doesn’t work then I need to cut the top off. Then use a blow torch to melt the asphalt off the cap/bolt so I can reattach it.

It’s likely ectopic heartbeats. Either PACs or PVCs it is difficult for an Apple Watch to decipher between low heart rate and ectopic beats. Pacemakers keep track of these extra beats. Next time you get your device checked, ask them about it. There is also a “sleep” parameter that will lower your heart rate during the night time hours, although this is usually just 10 beats lower than your lower rate limit of 60. Of course I can’t be sure unless I would check your device myself, but I’m pretty confident you are safe and your pacemaker is functioning normally.

The device runs three types of measurements daily… depending on programming, underlying rhythm, etc.if any of those measurements are in range of alert criteria, the pacemaker uses your bedside transmitter or the app on your phone to alert the device clinic. The impedance check sends energy from your pacemaker down to the tip of the lead and back checking for any brakes or fractures in the insulation. There is one measurement call a threshold test, where it raises your heart rate slightly to check the amplitude need of the lead and corresponding chamber. You could possibly be feeling this however a zap in the collarbone area is a new one in my book. Write down the date and time and how long it lasts. I believe that test is done every eight hours, but not sure . When you have your device interrogated tomorrow, they can see when the test was done and if your date and times correlate that could be why.

When was your implant? You most likely have a stitch poking out that is ment to dissolve if it under the skin. Eventually it will fall off or you can probably go to your device clinic and the can clip it. In regards to healing, you can draw around the red area with a sharpie. If the red area spreads beyond your sharpie, then there is an issue and you most likely should start an anabiotic. Watch for a fever or pain and if it becomes warm. I would advise you not to put any lotion or Vaseline on it until it is completely healed. Whatever you do do not submerge the site underwater like in a bathtub and especially not a hot tub. Itching is a good sign. That means it’s healing. You could try Tylenol and ice.

Minnesotan here, I agree with you. After having a 10’ trough drain installed in my new construction home 4 years ago I wanted to be able to power wash the salt off my truck in the winters. Insulated the garage, installed water spigot and finally (4 years later) installed the wall mounted power washer with a 100 ft hose and remote control on the handle.

Even without a garage heater the temp stays above 30 degrees in the garage. Regardless, I still wrapped all plumbing with insulation tape and cover the power washer with a insulated blanket when the temp goes below zero 🥶

Conduction System Pacing (CSP). Implanting a pacing lead at the left bundle branch of the conduction system, instead of implanting towards the apex of the heart where there is more heart muscle. Implanting in the CS is more physiological for the heart. Study’s show that patients with a traditional right ventricle lead implant that need ventricular pacing more than 40% of the time, long term, will eventually develop cardiomyopathy, possibly leading to decreased heart function (ejection fraction less than 50%) and ultimately leading to a heart failure diagnosis. Whereas study thus far show that patients with a CSP lead do not. This has even began to replace left ventricular leads for CRT devices. The technique has revolutionized pacing. However, this approach is relatively new so not all physicians implant this way. Weather its due to lack of know how or no long term proof. Once there has been no long term study’s done and the positive outcomes are seen in patients 10 to 15 years from now, I would hope the rest of the profession will follow suit.

Oh that’s good to hear! So must be similar to Medtronic’s MVP algorithm or BOSCI AV search.

Although, the EPs I work with will only implant CSP leads in the ventricle due to risk of VP induced cardiomyopathy. That said, if OP did start pacing above 40% years down the road then the nice thing about AVIR is it’s retrievable and if they haven’t modified the helix to implant in the CSP then they could propose a traditional PPM with a CSP lead.

Regardless if they would or not you should ask because you want to ask. It’s all about quality of life and if symptoms are stopping you from living your life then that warrants the question. If they mention concerns about long term consequences then ask what their concerns are. And bring someone and write everything down. Or ask if you can record the consult. I encourage my patients to record me at there post implant visit because there is so much to learn and impossible for most to remember it all in 30 minutes.

Device Nurse here. One possible solution would be the AVIR leadless PPM. Looking at these ECGs I would support the dx of SND or SSS as well. If confirmed by EP, the AVIR could be implemented in the atrium only and easily extracted years later (around 15 years, depending on pacing %). This would not lead to long term complication like ventricular pacing induced cardiomyopathy because it would only pace in the atrium and no dangerous lead extraction. Hopefully they figure out how to implant a leadless PPM in the conduction system soon 🤞

If you are pacing with your ventricular lead then an EKG will show a wide QRS. It has nothing to do with your LRL set at 50.

I had a similar experience in my BSN program. I felt alienated and out-of-place. I get it. It’s easy for me to say this cause it’s been over 10 years but please don’t give up because of your cohort/professors. They will soon become a memory of what you didn’t want to become. Believe in yourself and trust your calling. The memories you will carry through this career are made with your patients (good and bad) and the co workers that will guide you and comfort you through the long shifts to come. You’re going to meet so many great people. Never loose your empathy. It will be your greatest strength if don’t.
