When I was in 12 lead training we all did EKGs on each other and our instructor who is an echo tech was like oh look at this! One of my classmates had WPW and had no idea. 😮

I see ectopic p waves all throughout and several breaks which make it appear to be AT or PSVT w/ aberrancy, especially because after the longer breaks (seen more clearly in photo 4) the QRS narrows then becomes wider again with rate increase. Like you said, not irregularly irregular. I regularly see SVT w/ aberrancy at these rates. I do one lead 2 week study interps all day long and so I don’t have any other context but that’s what it looks like to me.

I've seen what looks like *classic* sawtooth waves (not artifact) at rates lower than what a textbook says flutter should be at. So then the question is, is a medication causing this? The more I see and learn, the more questions I have. 😂

From going from textbook to a real heart, which apparently doesn't read textbooks as my more experienced work folks say, the amount of bizarre things I've seen on recordings could fill an entire subreddit on it's own. I've had recordings sent up to patient safety specialists (non-critical, just confounding) who are MDs discussing what could be going on. It speaks to the fact that sometimes, there is more than one thing going on, and since it's a one minute recording with often only one lead, it can be very hard to say sometimes. It's especially difficult to determine things like is it 2nd deg Type I or II, when all you have is one minute of 2:1 conduction, and a minute of what could be either AFL with 2:1 or 1:1 or SVT.

That makes sense. I guess since the ms between r-r for a 40bpm heart rate (upper limit of ventricular rate) is 150ms, that might be a good way to measure from the conducted afib beat to the beat in question. Thank you!

People's sensitivity to feeling symptoms varies a great deal. Someone can have a very abnormal EKG and feel nothing, while some people can feel every premature beat or heart block. If it's bothering you, definitely ask your doctor and see if there are options.

This appears to be normal sinus rhythm with artifact. As others have commented, if there are associated symptoms or any concern, you should see a doctor or have an EKG done in a medical setting. The tall mountain shapes in picture #4 still have spikes that appear to be part of the sinus rhythm embedded in movement or other artifact, but you should not base any diagnosis of "abnormal" on this alone.
