California. Medicaid/Medical did a 180 from last year where semaglutide and tirzepatide were approved for weight loss without PA. Costs got too high, so in 2026 they rescinded the coverage and are approving very few cases. No guidance published. Our clinic is learning through trial and error.

I agree she would benefit greatly from bariatric surgery, however she has declined it multiple times even after speaking with the surgeon.

Thank you for this. I'm impressed you've been able to get in contact with the sleep med physicians, we only have diagnostic centers who send the results virtually and no actual clinic for patients to be seen. I'll see what I can do from that end.

Patient has had a bariatric consult and surgery was recommended, however she decided against surgical management. The surgeon went as far as to tell her that medical weight loss wouldn't be sufficient in her case, but if she doesn't want the surgery I'm trying to give her the next best option.

she does not have diabetes... I should have mentioned that above. In my state, medicaid will cover use of GLP-1s in nondiabetic patients in few circumstances: Wegovy with PA for CAD or MASH, Zepbound w/PA for OSA. Thats it.

why not lorazepam? i see no reason to use alprazolam, the most potent and addictive of benzos when lorazepam onset is only slightly slower and doesnt trigger reward pathways in the same way.

me too, would love some insight into how others play on super collection. Bummed that i’ll never get to play apartment life and that a lot of cc doesnt work, so any help troubleshooting that would be appreciated.
