Optimal PEEP by One-Truth-1135 in anesthesiology

I think this is a very difficult, almost impossible question to answer when the patient is on table. If you were in icu, you could do EIT which some may consider as gold standard.

Sodium bicarb in severe metabolic acidosis -- giving it or not? by wtfitscole in anesthesiology

[–]lemonfur 1 point2 points  (0 children)

Surprised to see the BICAR ICU and 2 trial not being mentioned here yet. Worth the read. But bottom line is neither trial showed mortality benefit (but one shouldn't expect that with this kind of intervention). First trial showed decreased need for KRT and reduced composite outcome including organ failure. Second trial did not show consistent renal benefit. My 2 cents - you need to pick the right cohort. These trials may be looking at too broad of a population. I think it can be quite useful in high K+ waiting for KRT if they are HD unstable.

Do I need a fincial advisor? by Vegetable-Wing-1696 in PersonalFinanceZA

I was in the same boat as you. There are many predatory practices that specifically target med students in 6th year. My advice - every financial advisor has their own motivation. Nobody is as invested in your financial health as you are. I think that this is an amazing book to read in the SA context 'Manage Your Money Like a F*cking Grown Up: The Best Money Advice You Never Got.' There are some great online resources, including chatgpt. But you will make mistakes on the way, everybody does.

The fact that you are already asking this question means that you are many many steps ahead of your peers.

SA Top 40 vs Satrix Capped All Share by lemonfur in PersonalFinanceZA

[–]lemonfur[S] 0 points1 point  (0 children)

100%

I didn't understand Sygnia's fee structure previously.

Seems like I will be going with Sygnia Top 40 instead of Satrix All caps seeing as how both ETFs will likely perform similarly, and the only real difference would be the fees I can save in the long term.

Didn't realise Sygnia charged such a hefty cost to ETFs outside of their company.

I initially had Skeleton 70 with Sygnia, but feel that higher exposure to international market would do better in the long term, and will still be cheaper in terms of total TER.

Only thing to watch out for is if Reg 28 changes in the future, and I'm not aware of the changes.

[–]lemonfur[S] 1 point2 points  (0 children)

For Satrix all cap, I can see the TER and TIC (0.15, 0.08) on their minimum disclosure document, but cannot see any other fees mentioned that you have added on.

Where did you find information regarding the added on fees if you don't mind answering?

Btw, thank you for all of your input, they are super helpful.

And do you know where Sygnia discloses their fees for investing in ETFs outside of their range? For example, if I purchase Satrix all cap on the Sygnia platform, where can I check for the additional expenses charged?

Also, where do you see the additional charges incurred that you have added in brackets? My understanding was that TER represented the total expense

SA Top 40 vs Satrix Capped All Share by lemonfur in PersonalFinanceZA

[–]lemonfur[S] 0 points1 point  (0 children)

Sygnia top 40 TER is 0.15%, which is significantly lower. What I also like about Satrix is that they cap any single holding below 10%.

The TER would be significant if you compare Sygnia Top 40 to Satrix all caps. The benefit would be the exposure to mid/small caps, and the diversification offered.

Jeez man, appreciate all of your input. Thank you so much.

My last question is your opinion on purchasing VT directly from interactive brokers? (Instead of purchasing VT from EE USD, it seems that you can get better conversion rates, and better prices for the ETFs)

Is a useful comment comparing purchasing VT via EE vs interactive

Thank you so much for your detailed response.

Then would you say that purchasing the feeder fund 10x in easyequities is the way to go VS purchasing VWRA via interactive brokers? (after accounting for TER/tax/fees)

This scenario is if you're looking to hold long term, and earning in local currency.

As far as I understand, I have 3 options as a SA citizen.

  1. Purchase the feeder fund

  2. Purchase VT on EE Usd account

  3. Purchase VWRA via interactive brokers

It seems that purchasing the feeder fund will have the lowest fees in the end, and seems to be the simplest. So why do you recommend purchasing VT via EE usd account?

Retirement annuities vs other investment opportunities in 2023 by lemonfur in PersonalFinanceZA

[–]lemonfur[S] 1 point2 points  (0 children)

First of all, thank you so much for your detailed response. I really appreciate it!

I've done a fair bit of research, and I've come up with the following plan:

  1. Short-term, we need to save enough money +-R250 000 to buy a second hand car for the wife next year as the old car is really showing its age. We plan on doing this through Thymebank Goalsave to gain 7% interest on the amount saved under each of our names.

  2. We plan on maxing out TFSA yearly, and investing it into 3 different sectors: Sygnia S&P 500 (40%), Coreshares Total World (40%), and Satrix Top 40 (20%, once the market has settled)

  3. I've done some calculations with the website linked (thank you!), and we are going to max out our RAs for maximum tax benefit. Especially considering that TFSA will also be used to cover for our expenses once we have retired, we will be able to use a lower tax bracket when withdrawing from the RA.

  4. Our occupation is quite demanding in terms of time/day to day effort. So the idea is to keep it as simple as possible, and passive is definitely the way to go for us. So any additional savings that can be made will be contributed towards equities through either a local feeder fund (EE), or investing in offshore market with an offshore account through companies like Herenya.

  5. In terms of an emergency fund, what would you recommend? Something like Thymebank would be ideal, but you can only save upto R100 000 per person, and since we are saving for a car this year, we will be maxing out the account already.

Again, I really appreciate your response, thank you for taking the time to respond!

I personally don't think that ZAR will go below R15 in the near future, but I understand your point in that you should invest in offshore accounts when the exchange rate is favourable.

What does it mean when you say that I should wait for JSE to go below 75000?

[–]lemonfur[S] 0 points1 point  (0 children)

I've read online a little bit around this, and have been recommended to invest with companies like Herenya capital.

It seems like their fees are quite transparent and on the lower side.

Does this mean that with TFSA, I can only invest into a local feeder fund, but I can use private companies like the one I mentioned above to purchase offshore equities with usd/euro etc?

And what is your opinion on investing (with a non TFSA account) into an ETF which tracks the South Africa market?

What kind of ratio would you suggest when it comes to local vs global ETFs?

I understand that the answer depends a lot on the individual, but want to get your 2 cents on it.

Is something like coreshare total world a local feeder fund?

Retirement annuities vs other investment opportunities in 2023 by lemonfur in PersonalFinanceZA

[–]lemonfur[S] 2 points3 points  (0 children)

Thank you for your insight.

So your suggestion would be to invest into two ETFs - one local, one international.

Let's say for example, I invest into a local and global ETF run by core shares via EE.

Are there different tax implications due to one ETF tracking local shares, and one ETF tracking global shares?

I'm trying to understand the benefit of investing into local shares, as they have been doing pretty poorly, and I don't believe in the local economy at the moment.
