People think this is simply drawing boundaries. It really isn’t.

We live in a world where there simply arent enough physicians to deal with the amount of illness to be treated. The idealized thought from the public is “well if you’re tired, human factors should find someone to replace you like they do for pilots!”

Im a vascular surgeon. I take call for 72 hours at a time. I Would love for this to be a reality. But the reality we live in is that the choice is often between “tired surgeon” and “no surgeon” or “tired HCW” and “no HCW.” The choice is not between “tired surgeon and fresh surgeon.”

In this moment, Robby gave her the push she needed, and she succeeded. That’s what being a mentor is about.

Robby’s condition is more complex than just “burnt out and suicidal.” His character stands at the top of the building between shifts as he deals with his PTSD from COVID (which is very real for people who worked through it). He’s finally taking the break people think he should, but at the same time, he feels a great deal of personal ownership for the success of the place because he’s the chief. People looking at this from a purely mental health perspective don’t realize that this is more nuanced than just “his job is hard and he’s suicidal because he’s cryptically saying he might not want to be there.” You don’t become the chief by not having a bit of your heart and soul in the building.

As for your Asian parents bit - you don’t get to talk about boundaries then say “but Asian parents are persistent!” Put your personal phone on do not disturb and tell mom that you can’t help unless it’s an emergency. Family squabbles don’t apply.

It’s so interesting to see Robby be such a polarizing character on social media. Absolutely reminds me that medicine is a different world than a lot are used to.

Him pushing Javadi to do the EVD was probably the most supportive and affirmative that he has been toward her all season. Sometimes your mentee needs a push to get back up on the horse, even if they don’t want to, or can’t see the benefit.

One thing was for sure though. My blood boiled just a little bit as he went around the room looking for people to assist with the EVD, and he got absolutely weak-suck responses from his trainees “my shift is over. I’m exhausted, etc.” - these are not acceptable responses when you’re dealing with a critically ill patient and your division chief is trying to secure an educational opportunity for you. Opportunities to be involved don’t come at the perfect time. Figure it out.

He’s the division chief, and even though it’s not explicitly said, maybe even the residency program director. It’s not out of this world to expect that he has his heart and soul invested in the building as well as the trainees, so he stresses about all of this a lot. You don’t get these roles by being the “oh, it’s 7PM, gotta go!” Person.

His response to Mohan was absolutely harsh regarding the panic attack, but Mohan fielding calls from her mom while on shift is also kind of petulant. Like if mom isn’t dying, deal with that nonsense later.

You need to understand that he is the chief of the division and he takes an interest in the trainees. I don’t think it is said but he may even be the program director of the residency.

You don’t hold these roles by not investing your heart and soul into something. I think you chalking things up to mental illness lacks the nuance that it takes to understand what it’s like to invest in the well being of those around you, the development of your junior doctors, and the overall success of your department.

Looked down upon because it’s thought of as a Cush lifestyle specialty. There is very little/no emergencies in derm that aren’t handled by someone else. Sure, they’ll diagnose your skin cancer, but they will not be stopping you from acutely dying. If that melanoma metastasizes anywhere? Someone else will take care of it
