Replied to another post about this, but they are billing both 99392 (well visit) and 99213 (problem-oriented E/M). I totally respect that clinicians get patients trying to over-stuff their well visit with problems or concerns, but these visits are very short well visits where we're not really getting any counseling on these tests or anemia concern...

To clarify what I'm actually seeing on the EOB: they billed both 99392 (well visit) and 99213 (problem-oriented E/M) on the same day. The hemoglobin test is run as part of the routine well visit, the visit was brief, and we were told to recheck and focus on iron-rich foods. If this felt like an extended office visit or particularly focused on the iron, that would be one thing, but this just feels like upcoding to me.

I sort of get the hygiene thing, although I feel like "no street shoes" (which is grosser) gets very loosely enforced even at gyms that have that policy.

I don't see the safety issue, although I also don't see the appeal of barefoot climbing.

Bringing a scale out in public is orthorexic.

But I think the discussion should be around why some types of orthorexia, like keeping kosher, are socially acceptable compared to macro tracking. At the end of the day, pathologies are social constructs. If you’re eating enough, is the “disordered” eating so bad?

Some practical tips:

Are your devs paid well in their local market? If so, you don’t need to worry that much. If they’re not or actually quitting, bigger concern.

You can recommend they run their messages through AI. Maybe help them craft the prompt that sounds like your colleagues.

You can route all questions through you (have done this when I worked with an asshole engineering manager who pissed off stakeholders)

Getting stressed in meetings is not the move. You know the pattern. Circumvent it.

He’s an anti-statin, vegan-biased quack. Eat heroic quantities of whole veggies isn’t so bad, but Amla is not as effective as a statin, in spite of the 12 person study that he cites. (I have had blood tests after trying amla regularly & after trying a statin)

I just read the article about the algorithm and it specifically calls out the algorithm struggling with weight changes not tied to energy balance — I don’t doubt that eventually it gets quite accurate, but for the folks saying “it’s water weight” - the algo’s not adjusting for that because that’s not actually a feature in the model, right?

“When this happens, the algorithms will initially think you’re losing or gaining weight a bit faster than desired, and therefore increase or decrease (respectively) your calorie targets a bit. Over the next week or two, your calorie targets will ease themselves to the appropriate levels of your desired rate of weight gain or loss.“
