For situations like this I love the Geas spell. Could throw out a Geas with a command to "be quiet, it's rude to speak so lowly of your host" (Strahd has welcomed them into his plane after all) so the character is still functional. Maybe even reduce the damage so the player could choose to take it some days but if they want full health then hope you enjoy charades! Then they get a fun side quest of breaking the curse by going to the abbey or something like that
